- Medication errors harm an estimated 1.5 million Americans each year, resulting in more than $3.5 billion in additional medical costs.
- About 1 in 25 U.S. patients suffer at least one infection contracted during the course of their hospital care.
- About 1 in 10 U.S. patients experience an adverse condition, such as a pressure ulcer or fall, during hospitalization.
- In the United States each year there are 611,100 deaths from heart disease, 574,800 cancer-related deaths, and 44,000 to 440,000 estimated deaths from preventable medical errors in hospitals [emphasis added].
- According to the World Health Organization there is a 1-in-1,000,000 risk of being injured during air travel while there is a 1-in-300 risk of being harmed while in a healthcare setting.
“Patient safety” is defined by the Institute of Medicine (IOM) as “the prevention of harm to patients.” Emphasis is placed on the system of care delivery that prevents errors; learns from the errors that do occur; and is built on a culture of safety that involves healthcare professionals, organizations, and patients.
